Step 1…. Look at the front of the insured card. Read it word by word (there won't be many) and see if it says PPo or. HMO or PO… if not, and this is very important, turn the card back. Read the back word by word (there are several) and look for a series of 10 numbers with a few dashes between. It will say something like "call customer service"


Again very important…. Use your phone to dial the numbers on the back of the card to get someone on the other end of the phone. Enter your policy number which is on the front of the card (you will need to turn the car back for this.)

Next, ask a customer service representative over the phone if your plan is an HMO or a PPO, or a POS… and they will tell you. While you're at it, ask relevant questions like doctor networks, deductibles, out-of-pocket maximums, and deductibles.
